The death of a loved one is a difficult and stressful life event. How do I go about submitting my claim?
To file a death claim, you need to complete the "Declaration of Rightful Claimant - Death" form, which one of our customer support officers will be happy to send you.
If you have a copy of the insurance policy, look for the name of the representative you can also contact for help with this process.
What documents do I have to provide?
You may be asked to submit various documents, depending on the circumstances and the designated beneficiary. However, birth and death certificates for the deceased are always required. In some cases, you may also be asked for medical reports and the deceased's will, his or her marriage contract or divorce decree (where applicable), or a declaration of heredity (Quebec).

Where and how do I obtain a death certificate?
In Quebec
Death certificates are issued by the Directeur de l'état civil. The "Request for certificate or copy of act" form is available at caisses populaires, Communication Québec offices, church rectories, CLSCs, courthouses, and funeral homes, or it can be downloaded from the Directeur de l'état civil Website.

In Quebec
If you haven't found the required documentation in the deceased's personal belongings, you need to do the following:
- To obtain a will, contact the Chambre des notaires du Québec or the Barreau du Québec.
- To obtain a birth certificate, contact the Directeur de l'état civil.
What do I do if the beneficiary of the insurance policy is deceased?
You must provide the death certificate for the deceased beneficiary. You may also be asked to provide his or her will.
